WIC Application Process
The following steps are involved in applying for the New Jersey Women, Infants, and Children (WIC) Program:
- Contact your local health department or the New Jersey Department of Health for an application form.
- Complete the application form and provide proof of income, residency, and identity.
- Attend a screening appointment with a health professional to determine if you and your child are eligible for the program.
- If approved, you will receive a plastic card called an EBT card that you can use to purchase food and formula at authorized stores.

Eligibility Requirements
